Medications

 

When a child’s physician prescribes medication to be taken during school hours, state law requires that the physician state the dosage and related information on the appropriate school district form, which is available in all school

When a child’s physician prescribes medication to be taken during school hours, state law requires that the physician state the dosage and related information on the appropriate school district form, which is available in all school offices. Parents are required to sign a release allowing school personnel to dispense medication (See Student Health Forms)

 

An information sheet about medication administration, illness, and communicable diseases is distributed in all student registration packets and is available by contacting your school principal or the district health office at (949) 234-9574 or click on the document below.

Oral Health Requirement for School Entry

An oral health assessment is required for children entering kindergarten or first grade. The parent/guardian must submit proof of an oral health assessment performed by a licensed dentist by May 31st of the first school year. (Ed Code 49452.8) The goal of this program is to establish a regular source of dental care for every child. The program will identify children who need further examination and dental treatment. Dental decay is the most common chronic childhood disease and is easily preventable. Dental decay affects nearly two-thirds of California’s children by the time they reach third grade. This new law is intended to raise parents’ awareness of the importance of oral health to overall health and readiness to learn.

Physical Exam (Health Check-up)

A health check-up (physical examination) is required for all children entering first grade. The district encourages parents to obtain the exam prior to or during the child’s kindergarten year. Parents can pick up

Vaccination Requirements

 

 

On June 30, 2015, Governor Brown signed a new vaccine Bill (SB277).  The law will take effect on July 1, 2016. This new law requires that all children in California be fully vaccinated to attend public or private school.  Fully vaccinated means the 10 required vaccinations* currently required by the state.

 

The District is required to implement the new law and starting in 2016, SB277 will:

 

  • No longer permit immunization exemptions based on personal beliefs for children in child care and public and private schools;
  • Remove immunization requirements for:
    • Students in home-based private schools
    • Students enrolled in an independent study program who do not receive classroom-based instruction
    • students with medical and personal beliefs exemptions from any new immunization requirement initiated by CDPH for attendance at school or child care
  • Allows for the enrollment of students who qualify**:
    • For an Individualized Education Program (IEP) to access special education and related services (federal law and Section 56026 of the Education Code)
    • As foster or and homeless youth (Section 48850 of the Education Code et seq.)
    • As conditionally admitted pending full immunization within the time periods designated by regulation of the California Department of Public Health

**Students in the above categories will still need to provide immunization records to their schools before entry.

Requests for Immunization Records need to be made by the student. Once you become 18 years of age, you must authorize the release of your Immunization record. Please be sure to sign your request.

Vision and Hearing

 

Currently in CUSD, all students in kindergarten, 2nd, 5th and 8th grade are screened by Bio-Acoustical Corporation for a vision assessment. Boys in second grade are additionally screened for color vision.  These are State mandated screenings. Since vision and hearing screenings may only be done by certificated or credentialed school nurses our eight District Nurses facilitate any additional screenings, which include:

       

  • All make-ups for mandated screenings
  • Vision and hearing parent or teacher concerns.
  •  All initial IEP’s and Triennial IEP’s for students preschool to age 22 years old, where vision concerns arise.
  •  All annual IEP’s for students with vision and hearing issues, students preschool to age 22 years old.

 

Additionally, District Nurses refer students to community providers and offer State funded Vision Service Provider (VSP) coupons to qualifying students (no Medi-Cal or insurance).
